Q: Why do you need my personal info?

A: So we can contact you by phone, snail-mail, and e-mail.  This is also used

     for our church database system.

 

Q: Why do you need another medical form from me?

A: Even if you have filled out a medical form for another church group,

     we need one for all AGEHR sanctioned activities as well.  These forms

     must be filled out every year, even if nothing has changed.

 

Q: I don't want anyone to know my medical status.  Who sees it?

A: Your medical form is confidential and will be in a sealed packet.  Other than

     myself (just to make sure everything is filled in) and the Notary Public (so they

     can see where to affix their seal and sign), nobody will see it.  The information

     is for any medical team that may need to treat you in an emergency.

 

Q: Why do you need my t-shirt size?

A: Part of your registration package at festivals includes a festival t-shirt.

 

Q: Why do you want to know my favorite color?

A: You'll find out when you graduate high school.

 

Q: What if I can't afford my share of money for our trips?

A: This is answered in the General Information document.

 

Q: Do I have to work the dinners and other fundraising events?

A: Unless there is a scheduling conflict, you are strongly encouraged to help

      in our fundraising efforts since you are a part of our group.  This is how

      we can afford to go places.

 

Q: I'm a director of a bell choir. Can I use any of your forms for my choir?

A: Certainly!
